ALL STUDENTS IN PRE-K, K, 1st, 3rd, 5th, and 7th***
***ALL IMMUNIZATIONS FOR EACH STUDENT MUST BE TURNED IN AT THE BEGINNING OF THE SCHOOL YEAR***

If your child arrives late to school, please have them sign in at the nurse's office before going to their classroom.
If your child needs to carry an inhaler, an epi pen etc...please contact the nurse, there needs to be a self carry form on file.
Any medications that need to be administered by the nurse needs to have the correct form on file and a doctor's prescription on file.
ALL medicine, over the counter and prescription, MUST be labeled with your child's name in a ziplock baggie.
